He was predeceased by: his father Harry G. Lewis. He is survived by: his mother Carolyn J. Lewis; his son Daniel W. Lewis Jr.; his sister Jennifer L. Kellough (Larry J. Kellough Jr.); and his nephews, Nolan G. Kellough and Ty J. Kellough. He is also survived by other aunts, uncles, cousins.
