He is survived by: his children, Warner Jr and Frayna DeLeeuw Jr; his wife Joan Van Kampen DeLeeuw; his daughter Elizabeth (Evan Burton) of New York City; his siblings, Ann (Robert Decker) of Louisville and Warner (Lynne) of West Olive; his siblings-in-law, Thomas Essenburgh of Holland and Lori DeLeeuw of West Olive; his mother-in-law Bernie Kampen of Holland; his siblings-in-law, Londa (Mark Kurburski) of Holland and Dan Van Kampen (Dana); his aunts, Marilyn Colman and Gladys Colman; and also, Nancy (John Bolt) of Holland, Gayle (Michael Grotenhuis) of West Olive, Dawn (Preston Kragt) of Holland, Janice (Scott Vanden Berg) of Holland, Kristi (Michael Dekker) of West Olive, David (Duke), John of West Olive and Faye Van Kampen of Holland. He is also survived by many nieces, nephews and cousins.
