He was predeceased by: his parents, John F. Smith and Mary Smith; and his wife JoAnne Smith. He is survived by: his children, Michael Smith (Laura), Mary Vantongeren (Don) and Kathy Smith (Jeff Dziadula); his grandchildren, Christopher Smith (Jessica), Emily Wesley (Mark), Chelsea Shirk (Chad), Erik Vantongeren (Abby), Elizabeth Smith, Madison Dziadula and Michael Dziadula; and his great grandchildren, Lucas, Aubrey, McKenzie, Maxwell, Walter and Mara.
In lieu of flowers, memorials may be made to Mother's Against Drunk Driving (M.A.D.D.).
