He was predeceased by: his siblings, Dorothy Newby, Lois Hunsberger, Grace Tate and Bob Smith. He is survived by: his wife Gladys Smith; his children, Wade Slasinski, Brad Slasinski and Karen Slasinski; his grandchildren, Elizabeth Smith, Dale Miller, Zachary Smith, Ezekiel Smith, Malachi Smith, Courtney Smith, Matthias Smith, Nicholas Smith, Kailyn Smith, Gabriel Smith, Noah Slasinski, Elijah Slasinski and Micah Slasinski; and his sister Ruth Ann Sigler of Bechtelsville, PA.

In lieu of flowers, the family requests that donations be made in Doug's memory to Hope Community Church.
