He was predeceased by: his mother Lillian Carrier; his wife Leslie Kozloski; and his sister Marie Reese. He is survived by: his parent Leonard Kozloski; his daughters, Carissa Walker (Pastor Brian) of Tekonsha and Heather Lanphar (Travis) of Polk City, FL; his stepchildren, Brett Haeck of Elkhart and Sheena Burch (Tom) of Union; his grandchildren, Samuel, Curtis, Phillip, Nathan, Cherise Walker, Isabel Lanphar, Tyrus Lanphar, Mary, James, Sophia, Dohnavin and Madisyn; his siblings, Ellen Lambert, Lisa Cox, Diana Kinley and Thomas Carrier; and also Debra Steinbauer of Union.
In lieu of flowers, memorial contributions may be directed to Debra Steinbauer to help with expenses.
