He was predeceased by: his wife Nancy; his son Terry; his parents, John and Mary; and his siblings, Kenny, Barbara, Bernadette, Rosey and Eugene.

He is survived by: his children, Deborah Perri, Richard Kozlowski, Jr. and Scott Kozlowski; his grandchildren, Christine Bishop, Harmoni Eggert (Dennis), Alexandria Gould (Billy) and Jared Kozlowski (Aubrey); and his great grandchildren, Brionna, Tre'Shon, Dennis and Anastasia.
