She was predeceased by: her father Elmer Cleyon (Marilyn Clark); her close friend Patrick Springer; and her sister Debra Kirk. She is survived by: her mother Pearl J. Clark (Harrison); her sons, Eric Forman (Jessica Vannatter) and Daniel Forman (Lisa); and her siblings, Vicki Derderian, Michael Clark, Patrice Peacock, Pam Otton and Paul Fulton. She is also survived by many nieces and nephews.
