He was predeceased by: his parents, Col. Warren G. and Mary G. Cook; and his brother Raymond. He is survived by: his siblings, Carol Ann Hamilton of Oscoda, Susan Davis of Oscoda, Mary Jo Samotis of Oscoda, Warren Cook, Jr. of Oscoda and Diane Cook of Oscoda. He is also survived by many nieces and nephews.
