He was predeceased by: his wife Bonnie; his son Patrick Foltz; and his friend Gerald Lade (Jerry). He is survived by: his daughter Terri Foltz; his grandchildren, Andrea, Ryan Root, Bonnie Griswold (Travis), Cody Dennis, Amanda Horsfall (Riley) and Seth Foltz; his great grandchildren, Blake, Hunter, Evelynn, Brayden, Landon, Mackenzie and Raigen; his brother Gary (Barb); and his friend John Fisher.

Flowers may be sent to the Church or you may make a donation in honor of Dale.
