He was predeceased by: his parents, Lewis D. Cook and Janet Tuttle Cook; his sisters, Sally Orvis (Arnold) and Jane Stockfisch (Harold); and his half-brother Jerry D. Cook.
He is survived by: his wife Anne L. Frier; his children, Jennifer Kelly (Steven), Jeremy Cook (Angie), Matthew Cook (Gina), Laura Eichenberger (Troy), Adam Cook (Anna) and Emma Cook; and his foster daughter Samantha Lummen. He is also survived by 17 grandchildren and one great-grandson.
