He was predeceased by: his parents, Wayne Dack and Rosemary Dack; his sons, Duane Paul and David Wayne Dack; his brother-in-law Terry Twiss; and his son-in-law Craig Ward.
He is survived by: his wife Patricia "Pat" Dack; his children, Penny Ward and John Dack; his grandchildren, Amber Halstead, Cara Ward, Mac Ward (Ashley), Paul Dack (Jenessa), Bradly Dack (Jessica) and Austin Ward; his great grandchildren, Cassidy, Aubrey, Kennedy, Callie, Addison, River, Esme', Jack and Henry; his sister Ruthann Twiss; and his nephew DuWayne Twiss (Dawn).
