He was predeceased by: his parents, Emil Polak and Estelle Polak (Buczek); and his sister Evelyn Reiss. He is survived by: his wife Helen Blaney; his children, Cindy Sauser (Don), Tim Polak, Nancy Palmer (Dave), Chris Davis (Dan) and Penny Polak (Lee Jerousek); his grandchildren, Nikki Harvey (Matt), Kari Davis (James), Alicia Jorgensen (Earl), David Palmer (Angie), Les Shaw (Jen), Nic Shaw (Erin), Jessica Cherry and Kalie Davis (Adam McArthur); his siblings, Linda Wildey (Al), Dennis Polak (Diane) and David Polak (Gerry); and also Mary Bratherton. He is also survived by 16 great-grandchildren.
