He was predeceased by: his son Frank Kootsillas, Jr. (Sherri). He is survived by: his spouse Debbi Kootsillas; his sons, Kevin Kootsillas (Lisa), Jeff Kootsillas (late Jennifer) and Corey Kootsillas (Kathy Shuel); and his siblings, Joan Tillman, Robert Kootsillas and Tom Kootsillas. He is also survived by his loving nieces, nephews and cousins.

Memorial contributions in honor of Frank may be made to the Capuchin Soup Kitchen and/or the National Wildlife Preservation.
