He was predeceased by: his sister Margaret Smith (late Vic). He is survived by: his wife Judith; and his children, Lisa Eubanks, Jacqueline Gainer and Derrick Costner (Leah). He is also survived by 8 grandchildren, 1 great grandchildren and numerous nieces and nephews.
