She is survived by: her children, Karen Mallett, Keith Formella (Heather Mason), Karoline Berg (Brandon), Kurt Formella (Sandy) and Kim Formella; her grandchildren, Steve, Abby, Noah, Kayci, Madelyn, Matthew, Alex, Sarah, Katelyn, Max and Rebekah; and her siblings, Tom Fugate (Sue), Tony Fugate, Coleen Kosarue and Chris Thompson. She is also survived by many nieces and nephews.
