He was predeceased by: his parents, Arthur Cook and Florence Cook (Olsen); his siblings, Arthur Cook Jr. and Barbara Galloup; his brothers-in-law, Fay Galloup and Jim Letts; his great-uncle Bob; and his aunt Ova Cook.
He is survived by: his wife Sandy; his children, Chad Cook (Denise) of Stanton, Patricia Martin (Glenn) of Greenville, Trina Speiser (Steve) of St. Joseph, Holly Kava (Phil) of Stanton, LeeAnn Reese (Randy) of Stanton and Lori Forton (Jim) of Sheridan; and his grandchildren, Nicole Phillips, Naomi Whitney, Salena Martin, Gavin Martin, Seth Williams, Anna Williams, Joe Williams, Jackson Speiser, Andrew Speiser, Eden Stephens, Alexandria Jeffery, Philip Kava Jr., Tracy Grider, Mathew Kava, Holly Reese, Molly Arellano, Rick Bracey, Christopher Bracey and Kaci VanDenToorn.
