He is survived by: his wife Karen Howard Cook; his children, Bryan Cook (Rebecca) of Sand Lake, Jason Cook (Elizabeth) of Grand Haven, Tara Escareno of Sand Lake and Brenna Alverson of Grant; and his brother Duane Cook (Gail) of Zeeland. He is also survived by several nieces and nephews.
