He was predeceased by: his parents, Frank Forner and Betty Forner (Gegoski); and his pet Smokey. He is survived by: his siblings, Dora Forner, Beth Parker (Rob), Norm Forner (Sandy) and Steven Forner; and his nieces and nephews, Janet Hulett (Kyle), Glenn Parker (Carly), Scott Berzley (Alessandra), Alex Berzley (Megan), Amanda Forner, Matthew Berzley and Adam Parker.
