He is survived by: his parents, Patricia Forsythe and Randy Forsythe; his brother Jeremiah Forsythe (Kristi); his nieces and nephews, Dustin and Jocelyn Forsythe; and his grandmother Mary Southworth. He is also survived by several aunts, uncles, cousins and many close friends.
