He was predeceased by: his wife Barbara; and his parents, Perry Teolis and Rose Teolis. He is survived by: his children, David (Kim), Perry (Laurie), John (Lisa), Maryrose Ann (Gary), Steven (Debbie) and Patricia Czech (Bob). He is also survived by ten grandchildren.

In lieu of flowers, donations made to St. Jude Children's Research Hospital.
