He is survived by: his wife Sandra Ellsworth; his children, Heather Ballien (Kurt) of Alma, MI and Joshua Ellsworth (Carol) of Hubert, NC; his grandchildren, Cameron, Adam, Devin, Kearstyn (Joseph Spencer), Noah and Liam; and his siblings, Hiram "Terry" Ellsworth, Jr., David Ellsworth and Pamela Graham (Thomas). He is also survived by many nieces, nephews, relatives and friends.
