He was predeceased by: his mother Margaret Kolakowski; his sister Nancy Fogarty; his grandson Jakob Kolakowski; and his parent Thomas Kolakowski Sr.. He is survived by: his wife Maria; his children, Kelly (Kevin Wright), Thomas (Alika) and Nancy (Robert Wisler); his grandchildren, Victoria (Devante Woods), Rachael, Gage, Jack, Luke and Austin; his great grandchildren, Logan and Ja'Kai; and his siblings, Kenneth (Micheal) and Mary Margaret (George Lindenberg). He is also survived by nieces and nephews.
