She was predeceased by: her parents, Curtis Cousino and Eleanor Cousino (Nick Rizzo); and her sisters, Lois Atwood and Rita Fortune. She is survived by: her children, Tony Rizzo (Sue), Jeanne Palazzolo (Chris) and Cindy Bozynski; her grandchildren, Matthew (Lisa), Nicole (Damon), Kimberly, Stephanie, Alexandria, Lindsay (Casey), Brittany (Dan), Tracy, Renee and Natalie; and her great grandchildren, Dominic, Aliza, Yena, Gabrielle, Sofia, Cain, Quinn, Charlotte and Lila.

In lieu of flowers, donations in Phyllis's memory may be made to Leader Dog.
