He is survived by: his children, Frank Kozlowski (Jessi) and Kathy Spangler; his grandchildren, Brian Nolan, Patrick Kozlowski, Alyssa Spangler and Johnny Spangler; and his brother Hank Kozlowski. He is also survived by many nieces, nephews, cousins, and friends.
