He was predeceased by: his parents, Sylvester Kubiak and Helen Kubiak (Zakalata); his son Patrick Sylvester Kubiak; his sisters, Kathleen Emery and Jean Downing (Steve); and his sister-in-law Katrina Kubiak.
He is survived by: his daughters, Christina and Helen; his siblings, Michael Kubiak, Syl Kubiak (Cheri) and Daniel Kubiak; and his brother-in-law Bill Emery. He is also survived by many nieces and nephews.
