He was predeceased by: his parents, Leonard Cook and Valeta Cook Sr. (Snow). He is survived by: his wife Linda Cook; his children, Greg Cook (Danielle Leithauser), Jeff Cook (Amanda) and Nikki VanDaele (Rich); his grandchildren, Parker, Aidyn, Micah, Grace, Mikayla, Diego, Lily and Rylie; and his sisters, Valerie Hall (Alwin) and Susan Josephson. He is also survived by numerous nieces and nephews.
In lieu of flowers, memorial contributions may be made to Spring Life Church or American Cancer Society.
