He was predeceased by: his stepmother Sally Perry; and his grandparents, Edna Gallagher, Delmont LaFountain, Ralph and Joyce Whittaker.

He is survived by: his parents, Phillip Perry and Cynthia LaFountain; his cousin Matt; his sons, Eric K. Perry and Gavyn Perry; his siblings, Amy (Robert Fraker), Chad (Jessica Perry), Heidi Ruppert (Pat McGee) and Kyle (Denise Fullhart); and his grandfather Ronald Gallagher. He is also survived by many aunts, uncles, nieces, nephews, cousins and special friends.

In lieu of flowers, memorials may be made to his sons.
