He is survived by: his wife Debra; his sons, Kenneth Davis (Stacy) and Andrew Davis (Melisa); his grandchildren, Logan, Carl, Haley, Luccas, Jadyn, Mason and Aubrey; his father-in-law Ken Vailliencourt (Jan); and his niece Deb Kalinin.

Memorial donations in Sam's honor may be made to the National Kidney Foundation.
