He is survived by: his wife Sheryl; his children, Tony Baciak, Jr. (Jenn), Tommer Baciak (Meredith) and Teresa Baciak (Whitney Lessor); his grandchildren, Anna, Gunnar, Gracie, Camryn, Cassidy, Maddie, Ryan, Jake and Cole; his siblings, Jack Baciak (Cathy), Jim Baciak (Roberta), Joe Baciak, Frank Zychowski (Sue), Margaret Baciak and Barbara Hahn (Frank); and his siblings-in-law, Michael Milligan (Karen), Lynn Opolka (Jim) of Norway and Diana Milligan of Niagara. He is also survived by many nieces and nephews.
