He was predeceased by: his parents, Joseph Hajek and Marie Hajek (Misek); his wife Mary Ann Mikula; and his siblings, Bessie, Frank and Anna.
He is survived by: his sons, Kim Hajek (Debra) and Chris Hajek (Rhonda); his grandchildren, Corey (Casey), Tonya (Travis), Megan, Hannah (Dan) and Carrie Ann (Neil); his great grandchildren, Haleigh (Jack), Leah (Tate), Ryder, Ellary, Parker, Noelle, Dominic and Ariana; and his friends, Joni (Greenlee) and Susie. He is also survived by his children and grandchildren and great-grandchildren.
