He is survived by: his wife Nancy Forker (Skalecki); his sons, Mike Forker (Shariann) of LaPorte, IN, Kevin Forker (Krista) of Columbia, MD and Steven Forker (Kris) of LaPorte, IN; his grandchildren, Brenton, Justin, Logan, Cambrie, Adriana and Isaiah; his great grandchild Marcella Rose; and his siblings, Debbie Graack (Ron), Diane Forker, Dawn Nowicki (Ted), Doug Forker (Cristel), Dorothy McDiarmid, Max Forker and Tommy Ryan.
