He is survived by: his wife Shannon Polen; his children, Damian Polen and Cheyenne Polen; his granddaughter Khloe Krebs; his parents, Byron Polen and Twila Polen; and his siblings, Beverley R. Polen, Butch Polen and Chuck Polen. He is also survived by several nieces and nephews.
