He was predeceased by: his parents, Matthew Johnson and Dianne Johnson. He is survived by: his children, Trystan Puhalski (Dakota Baker), Damyan Puhalski and Jacee Puhalski; his grandchildren, Jax and Oaklie; his sister Melissa Puhalski (Keith Sanders Sr.); his nieces and nephews, Jalecia and Javen; his girlfriend Amber Grey; and his pet Trouble. He is also survived by many extended family and friends.

Donations are being accepted to help the family with funeral expenses.
