She was predeceased by: her parents, Ben John and Henrietta Harjer (VanZuthem); her husband Bob Forrest; and her grandson Austin Vanderweele. She is survived by: her children, Michael Forrest (Mary Catherine), Karen Ayers (Michael), Lori Vanderweele (Bill) and Kurt Forrest; her daughter-in-law Cathy Forrest; her grandchildren, Adam Forrest (Rachel Watson), Elizabeth Collyer (Scott), Allison Wiseman (Chase), Waylon Carlson (Nickie), Matthew Carlson (Brittany), Nathan Ayers (Kathleen), Kaitlyn Drelles (Jacob), Braden Vanderweele, Holden Vanderweele (Julia), Alex Forrest (Julia Kwapiszewski) and Luke Forrest; and her precious kitty Polly. She is also survived by 12 great-grandchildren; many nieces and nephews.
