He was predeceased by: his parents, Richard D. and Patricia; and his brother Michael Patrick Kosciolek. He is survived by: his sons, Matthew Kosciolek (Amanda) and Eric Kosciolek; his grandchildren, Leah and Jonathan; and his siblings, Linda Bemis, Kevin Kosciolek and Gregory Kosciolek. He is also survived by many Nieces and Nephews.
