He is survived by: his parent JoAnn Cook (Mike Root); his sisters, Kelli Cook, Samantha Revels (Jake Lifford) and Phoebe Revels (Steve Ackerman); his nieces and nephews, Owen, Pearl, Lydia, Emma, Evyn and Vivian; his grandfather Roy "Papa Roy" Cook; his friend Connie Major; and his close friend Casey Mayes.
