He was predeceased by: his parents, Jay Wellington and Phyllis Jacoby (Weber); his son-in-law Jeffrey Tuttle; and his brothers, Gary, Jay W and Roger Jacoby.
He is survived by: his wife Dixie Campbell; his children, Dave Jacoby, Jr. (Tina) and Stephanie Tuttle; his grandchildren, David Jacoby III, Ashley Inman (Chris), Levi Finney and Lucas Finney; his great grandchildren, Faith, Payton, Gavin, Brody, Jaxson and Deegan; and his sister Sandra Baker (Chuck). He is also survived by many nieces, nephews, and cousins.
Memorial tributes may be made to the American Alzheimer's Association.
