He was predeceased by: his brother-in-law Lyle Merriam. He is survived by: his parents, Curt Petrak and Joan Petrak; his children, Chase and Addison; the mother of his children Danielle; his child Taylor Johnson; his siblings, Christopher and Jane Merriam; his niece Jeanette Cantu (Spencer); and his nephews, Mitchell Merriam, Allen, Owen and Dale Petrak. He is also survived by numerous aunts, uncles and cousins.
