He was predeceased by: his parents, Rex Cook and Gladys Cook (Henry); his daughter Debra Bruystens; his siblings, Raymond Cook (Mary), Ronald Cook (Beatrice) and Beverly Matice; and his sister-in-law Nancy Cook.
He is survived by: his former wife Patricia Ann Dane; his siblings, Leo Cook (Betty), Robert Cook (Kaye), Dave Cook, Roger Cook (Marion), Florence Holroyd (Duane) and Glen; his siblings-in-law, Pat Dane and Lewis Matice; his wife Pat Cook; his daughters, Brenda Lewis (Mark) and Sandy Cook (Robert Grimes); his son-in-law Millard Bruystens; and his grandchildren, Denis, Phillip Bruystens, Brian, Cory Wiggins and Cassie Lewis. He is also survived by 9 children.
It is suggested that memorial contributions be directed to St. Joseph County Animal Control for the construction of the new Animal Control Building.
