He was predeceased by: his parents, Herman Perry Cook and Margaret Cook (DeMotte); and his brother Gerald Cook. He is survived by: his wife Mary Hills; his sons, Kevin Cook (Michelle) and Greg Cook (Kathy); his grandchildren, Joshua Bloom (Madison Navarre), Emily Cook (Chad Compagner), Jessica Cook (Evan Miller) and Katie Cook; and his pets, Jinx, Abby and Sadie. He is also survived by many nieces and nephews.
