He is survived by: his daughter Kelly Koolstra of Punta Gorda; his grandchildren, Kayla Dage, Lily Church and Shaun Church; his son Jay Koolstra (Jennifer) of Lake City; his grandchildren, Marina, Mya, Dean, Liam and Sky Koolstra; his stepchildren, Renee Herrmann, Gary Herrmann, Jason Herrmann and Crickett Hermmann; his sisters, Hazel Stolte (Don) of Arkansas and Rose VanHaitsma of McBain; and his parents-in-law, Bernice Koolstra of Lake City and Deb Koolstra of McBain.
