He is survived by: his sons, Matt Perkins and Mike Perkins (Tammy); his grandchildren, James Blanchard, Ashley Blanchard, Cassandra Perkins, Victoria Perkins and Dylan Hollon; his great grandchild Wyatt Blanchard; and his siblings, Bobbi Shade of Van Martinez and Mark Perkins.
