He is survived by: his father Athol Hazen; his son Christopher Hazen (Melissa); his stepchildren, Jacqueline Razkowic and Michael Razkowic; his granddaughter Bella Hazen; his stepgrandchildren, Tori Razkowic (Derek), Dominic Razkowic, Alex Razkowic, Gianna Razkowic and Trinity Razkowic; his siblings, Mary Lyke and Richard Hazen; and his close friends, Tonya Moon Jettner and Joe Leichliter.
