He was predeceased by: his parents, Susie Hazen Sr. (Albright) and Athol Hazen; his wife Helen Hazen; his children, Lois J. Hazen and Leslie J. Hazen; and his brothers, Jay Hazen and Harold Hazen.
He is survived by: his children, Mary E. Lyke of Florida and Richard L. Hazen (Theresa Allen) of Marshall; his grandchildren, Chris Hazen (Missy Eich), Nathaniel Hazen, Zachary Hazen and Sarah Bray (Ben); and his great grandchildren, Bella Hazen, Evan Hazen, Johnathan Hazen, Kyle Bray and Gabriel Bray.
