He was predeceased by: his parents, Robert LaFave and Leona LaFave (Krouth); his stepfather Leon Rochon; his daughter-in-law Kate LaFave (Kariniemi); and his siblings, Robert "Beano" LaFave, Bobbie Cardew and Dale LaFave.
He is survived by: his wife Christine Forstrom Milliner; the mother of his children Janice Lahti (Steven); his children, Michael LaFave (Dawn), Laurie Olivier (Ernie Lindsey), John LaFave, Christopher Milliner, Richard Milliner (Heather) and Joe Milliner (Christine); his grandchildren, Taylor, Zachary, Madison, Sarah, Jack, Tom, Emma, Francine, Jessica, Skylar, Mackenzie and Joe; and his sisters, Carol "Jeannie" Bridergroom and Leona "Tootsie" LaFave. He is also survived by several great-grandchildren.
In lieu of flowers, memorials may be made in Jack's name to St. Jude Children's Research Hospital, 501 St. Jude Place, Memphis, TN 38105.
