He was predeceased by: his parents, David Middlestadt and Nellie Faye Middlestadt (Allred). He is survived by: his wife Tina May Middlestadt (Johnson); his sons, Andrew Middlestadt of Marcellus and Aaron Middlestadt of Marcellus; and his brothers, Jeff Middlestadt (Donna) of Schoolcraft and Dan Middlestadt (Jennifer) of Three Rivers. He is also survived by many nieces and one nephew.
In lieu of flowers memorial donations may be directed to the Funeral Home to offset costs.
