She is survived by: her husband Richard; her children, Michael and Dawn Chuba (Thomas); her grandchildren, Ashley Amthor-Shaffer (Bryce), Ezra Ketchum (Arika), Randall and Kellie Chuba; and her great grandchildren, Emory and Hadley Amthor-Shaffer.
