He was predeceased by: his parents, Floyd Cook and Irene Cook (Jones); his wife Janice Cook; and his children, Craig Cook, Jeff Cook and Mary Catherine Trent.
He is survived by: his children, Lori Brown of St. Johns, Connie Thelen of St. Johns, Pamela Mally (Mitch) of North Carolina, Tina Theis of Lyons and Kevin Cook (Melody) of DeWitt; his grandchildren, Douglas Cook (Tami), Corey Cook, Alicia Drake, Jessica Thelen-Thomas, Jami Thelen (Erin), Lindsey Terronez, Zane Crawford (Brandy), Ashley Reynolds (David), Shannon Crawford (Chris Carr), Brittney Trent, Amber Trent, Heather Sperry (Bryan), Victoria Keener, Brandon Theis (Erica Sorenson), Justin Theis (Hadley Clayton) and Kelsey Cook (Zach); and his siblings, Jerry Cook (Sandy) of Belding, Donna Marshall (Bill) of Gowen, Roger Cook (Liz) of Fowler and Kendall Cook of Saranac. He is also survived by many great grandchildren.
