He was predeceased by: his father Leon Frederick; and his brother Ken Proulx. He is survived by: his mother Nancy Jane Proulx (Prater); his son Samuel L. Proulx of Phoenix, Arizona; his brothers, Ted Proulx and Jon Proulx (Kim) of Raleigh; his sister-in-law Joleen Proulx of Minnetonka, Minnesota; his nieces, Jeniffer Bishop, Lexi Proulx and Aly Proulx; and his great-nephews, Jackson and Harrison. He is also survived by numerous aunts, uncles, cousins, and friends.
