He was predeceased by: his father Frank Kokoczka; and his sisters, Mary Ann and Patty. He is survived by: his mother Irene Kokoczka; his wife Alice Kokoczka; his sons, Sam Kokoczka (Elizabeth) and Matt McDonald (Erin); his sister Fran Galandi (Frank); and his nieces, Lisa and Crissy.
